Office Expectations at Stock HealtH:
At Stock Health, we believe that clear communication is key to establishing and maintaining a strong relationship while providing quality care and services. We understand that transparent communication enables potential clients to make informed decisions. Therefore, we have outlined our office expectations to ensure a straightforward process for your visit.
Before Your Visit
With you in mind, we've created these expectations to make your experience as smooth and efficient as possible:
Documentation
Bring a valid ID and insurance card (if applicable).
Prepare any medical records or history that might be relevant to your visit.
Payments due at time of service for all
Age Requirements
• Ages 18 and Older: We provide services for clients aged 18 and older.
• Ages 17: If you are 17 and will turn 18 in a couple of months, we can accommodate your needs.
Medication Services
• Same Day Services: We offer same-day services during office hours after insurance is verified for medication initiation, refills, or bridges.
• Ongoing Care: We will continue to support clients however, participation in your care required
• Restrictions: Stimulants, Benzodiazepines, or any medications deemed high risk for dependence are prescribed at providers discretion
Appointments
• Medication Assisted Treatment (MAT):
• Services Offered: We offer MAT for opioid and alcohol use disorders, including medications like Buprenorphine, Suboxone, Naltrexone, and Vivitrol.
• Late Arrivals: If you are more than 15 minutes late for your MAT appointment, it may be canceled or rescheduled. Please contact our office to reschedule.
• Initial In-Person : MAT services are provided initially in person unless acceptable barriers. Telehealth for MAT services are available when client has remained stable and providers discretion.
• Early Refills: No early refills are allowed for MAT clients.
• MAT Clients: Please schedule appointments online or call the office to schedule
• Repeatedly missing appointments (3 consecutive) without notification may result in incurring a fee or discharge from our practice requiring you to restart the intake process.
• Arrive at least 15 minutes before your scheduled appointment to complete any necessary paperwork.
• Walk-In Mental Health Services:
• Prompt mental health services are available for same day services during operating hours
• We do not offer crisis management services. Please report to ER for crisis also may contact 988
Psychiatric Evaluations
• We do not provide psychiatric evaluations for employment, medical clearance, animal support, disability claims, or legal matters.
Medication Safety
• In-Person Initiations: Medication initiations are done in person for your safety. However, Telehealth can be considered situation permitting
• Urine Samples: May require urine sample during visits to ensure appropriate medication prescription for your well-being.
• This is not for legal purposes or judgment but for safety. If you cannot provide a urine sample, medications may not be prescribed.
Records Requests
• You may request a copy of your records via our website.
• To provide you with the best care possible we may ask consent to discuss and collaborate care with other providers.
