Office Expectations at Stock Health:

At Stock Health, we believe that clear communication is key to establishing and maintaining a strong relationship while providing quality care and services. We understand that transparent communication enables potential clients to make informed decisions. Therefore, we have outlined our office expectations to ensure a straightforward process for your visit.

  • Payment is required at the time of your appointment/service. This ensures we can provide efficient, uninterrupted care and helps streamline your visit. We accept all major forms of payment and our team is here to help you understand your options before your session begins.

  • Ask us about our payment plans! We currently accept Carecredit, Apple pay, and After pay.

  • With you in mind, we've created these expectations to make your experience as smooth and efficient as possible. Please complete documents required to avoid delays or interruptions in care. Take your medications as prescribed unless otherwise instructed. Call office if not sure

  • Bring a valid ID and insurance card (if applicable).
    Complete any consents , intake forms, or questionnaires prior to your appointment that might be relevant to your visit.

    Documented ADHD confirmation required for treatment with stimulants. May complete psychometric testing

  • • Ages 18 and Older: We provide services for clients aged 18 and older.
    • Ages 17: If you are 17 and will turn 18 in a couple of months, we can accommodate your needs.

  • EXOMIND is designed for individuals experiencing depression. Consult with your healthcare provider to determine if EXOMIND is the right option for your mental wellness journey.

    Does it hurt?

    Most patients find the therapy comfortable. Sessions are quick, lasting under 30 minutes, and allow you to walk in and walk out with ease.

    Will it disrupt my daily life?

    EXOMIND seamlessly integrates into your daily routine with minimal disruption.

    What are the risks and contraindications?

    As with any medical procedure, there are some risks associated with EXOMIND therapy. Please discuss risks and contraindications with us directly before beginning treatment.

  • •Services: We can offer same-day services during office hours after insurance is verified for medication initiation, refills, or bridges.
    • Ongoing Care: We will continue to support clients however, participation in your care required
    • Restrictions: Stimulants, Benzodiazepines, or any medications deemed high risk for dependence are prescribed at providers discretion

  • • Services Offered: We offer MAT for opioid and alcohol use disorders, including medications like Buprenorphine, Suboxone, Naltrexone, and Vivitrol.
    • Late Arrivals: If you are more than 15 minutes late for your MAT appointment, it may be canceled or rescheduled. Please contact our office to reschedule.
    • Initial In-Person : MAT services are provided initially in person unless acceptable barriers. Telehealth for MAT services are available when client has remained stable and providers discretion.
    • Early Refills: No early refills are allowed for MAT clients.
    • MAT Clients: Please schedule appointments online or call the office to schedule

  • • We do not provide psychiatric evaluations for employment, medical clearance, animal support, disability claims, or legal matters.

    Please be advised you need to be established with Stock Health (more than 3 visits) and medication stable that have not required recent adjustments for refills

    Repeatedly missing appointments (3 consecutive) without notification will result in discharge from our practice.
    Arrive at least 15 minutes before your scheduled appointment to complete any necessary paperwork which can allow ample time the visit

  • • In-Person Initiations: Medication initiations are done in person for your safety. However, Telehealth can be considered situation permitting
    • Urine Samples: May require urine sample during visits to ensure appropriate medication prescription for your well-being.
    This is not for legal purposes or judgment but for safety. If you cannot provide a urine sample, medications may not be prescribed.

  • • You may request a copy of your records via our website.
    • To provide you with the best care possible we may ask consent to discuss and collaborate care with other providers.
